DL400D	Certificate of Completion of Behind-The-Wheel Driver Training
OL392	As of June 2010 this form is NO LONGER required by the Department of Motor Vehicles to enroll in Behind-The-Wheel Driver Training.

Lost Certificates:

All certificates issued by Economic Driving School are logged and audited by the California Department of Motor Vehicles. There is a $25.00 fee for a duplicate completion certificate.

If you lose your certificate do the following:

For Driver Education:
Send: Full name, address, city, zip code, approximate date and location you completed the course, a self addressed stamped envelope, check for $25.00 duplication fee made out to: Economic Driving School.

For Driver Training:
Send instructor’s name & approximate date you completed the training.

Your duplicate certificate will be mailed back to you within 24 hours from the day it was received.
